Following recovery from acute MI, these are markers for depressed ventricular function and increased mortality

  • Frequent PVCs (typically >10PVCs/hr)
  • Repetitive PVCs with couplets
  • Non-sustained VT
  • Reduce the risk of VF
    • BB
    • Correction of hypokalemia and hypomagnesemia
  • Antiarrhythmics (eg, lidocaine)
  • Amiodarone – reduces sudden death but not mortality
  • ICD
    • Reduces mortality in certain high-risk groups
      • Survived >5 days and have a reduced LVEF, non-sustained VT
      • Survived >40 days and have a LVEF of ?0.30
      • Have LVEF of ?0.35 and have symptomatic HF
    • Does not improve mortality when routinely implanted soon after MI.
