Drug toxicity

Nitrofurantoin

Both short- and long-term use can result in lung injury.

Acute pulmonary injury

  1. Short-term use
  2. Delay of 3-9 days from medication initiation to the onset of symptoms.
  3. Acute hypersensitivity involving the lungs
    • Basilar crackles, bilateral & basilar opacities
    • Pleural effusions , typically unilateral
    • Erythematous, maculopapular rash
    • Leukocytosis, eosinophilia
  4. Cessation of the drug
    • Typically leads to symptomatic improvement within 1-2 days.

Interstitial lung disease

  • Long-term use

Amiodarone
