Melanoma

Diagnosis

Skin biopsy

Incisional (partial) biopsy

  • may be performed for certain lesions (eg, on the face/digits/that are large in size)
  • If the full thickness of the lesion is not captured on biopsy, accurate melanoma staging may not be possible.

Full thickness exicional biopsy: preferred for all suspicious lesions

  • should include:
    • The full depth of the lesion
    • 1-3 mm surgical margins

Elliptical excision with 1-cm margins

  • Not recommended as the initial diagnostic test.

Treatment Techniques

Primary lesion

Cranial nerve tracking

Widefield RT skin protocol

  • 45Gy field dose, with 55-60 Gy SIB
  • Lymphatic “sparing strips” incorporated(?)

Parotid area LN metastases

Parotid 침범에 따른 P staging도 있다..?

Axillary metastases

Ilioinguinal LN metastases
